The Community Health Needs Assessment (CHNA) is a systematic and shared process for identifying and analyzing community needs and assets. The CHNA helps determine which critical health needs the community will focus on changing for the next 3-5 years. Please submit up to 3 questions for Dr. Person related to the chosen health priorities or CHNA process and she will do her best to answer all of them at the Facebook Live event.
