Community and Family Health encompasses services that  include care coordination, referrals, linkages, systems building and policy development for programs that focus on children and youth with special health care needs, children and young families, youth marijuana/tobacco prevention, as well as chronic disease prevention and management. 

Community and Family Health uses collective impact efforts throughout the counties to support our efforts. The Health District coordinates the Benton-Franklin Feeding Team for infants with complex feeding issues and leads discharge-planning efforts for infants from local area hospitals. Additionally, the Health District leads the ACEs and Resilience Collaborative, the Healthy Youth Parents Collaborative and partners with numerous coalitions throughout the community.
