Benton-Franklin Health District (BFHD) is currently conducting a Customer Service Survey in order to ensure the highest quality and standards in our programs, services, and practices.

If you have had a recent interaction with one of our staff members, whether it be at one of our locations or in the community, we would love to hear how your interaction went.

Here are some answers to common questions about customer service surveys:

Q: How long will it take?

A: It should take less than 5 minutes, but can vary slightly depending on the service you received.

Q: Do I get anything for taking it?

A: No, there is no direct incentive for completing the survey. The feedback, however, may help us to improve our services, which would benefit you and other members of the community in the future.

Q: How long do I have to take the survey?

A: The survey will be open until end of day June 30, 2022.

Q: Will my case manager/inspector see what I say?

A: No, the surveys are sent only to our Performance Management Department that handle our quality assurance and improvement projects.

Q: Can I take it more than once?

A: Yes, you can take it for each service you received.
